SIOUX FALLS, SD (KELO) – When it comes to health care, Sanford Health wants to do more than just take care of physical well-being, which is why they recently launched a website to meet the socio -economics of their patients.

Last month, the health system partnered with findhelp.org to create the Sanford findhelp website. It’s a vision that took nearly three years to come to fruition, according to Director of Nursing Erica DeBoer.

While working with care teams across the region, DeBoer heard managers say they spent a lot of time managing community resources to help patients meet other needs outside of the health system. . So she started trying to better understand what they were going through.

“What were those things that they had to spend a lot of time doing to try to meet the needs of our patients? DeBoer said.

Some of these needs could include food assistance, transportation, financial assistance for medications, preschool assistance, or other social determinants that affect a person’s health. Services offered on the new website are free or discounted and cover options that might not be included with Medicare or Medicaid.

Lindsay Daniels is Executive Director of Care Management System at Sanford and has experience as a field care manager. Daniels said that in one-on-one conversations with patients, she was often confronted with issues the patients were facing that went beyond what she could do for them during their appointment. This site helps care teams meet those needs, she explained.

“Knowing who provides what service when a patient was in front of me and needed to know how they were going to put food on their table at the end of the day, and also knowing that they needed to have some type of diet specific, perhaps, for the type of condition they had or how they were going to pay for their medications as well as the food that arrived at the door.

How it works?

The website is live and can be accessed through this link or through the MyChart app for patients. Once on the site, all you have to do is enter your postal code and select the category that suits you. After that, you will be provided with a list of resources with information on what the organization does, how to contact them, their hours of operation, and a map showing where the resources are in your area.

When a patient visits a clinic, Daniels said he might mention in conversation that he was having trouble paying his bills and that might affect what medications he could afford. In this case, the healthcare team could provide the patient with financial resources on their post-visit summary, direct them to the Find Help site, or make a referral on the patient’s behalf.

“It weighs a bit heavy on the hearts of the staff when they don’t have an answer for the patients,” Daniels said. “So this ability to just refer them to the website that’s printed on their [After Visit Summary] is a great way to do it.

The database includes hundreds of resources for patients and will only grow as it is used, according to DeBoer and Daniels.

Daniels explained that not only can healthcare teams direct patients to resources, but they can send referrals to community agencies with the patient’s consent. This means that if a patient wishes to be connected to a food pantry, for example, the Sanford care team would reach out on behalf of the patient and provide the food pantry with the best way to contact that patient and in the language that ‘he prefers.

“It puts all the stress on us and not the patient to have to log in and follow those referrals,” Daniels said.

DeBoer said the vision behind the website is Sanford’s goal of addressing the social determinants of health and how a person is able to care for themselves.

“COVID has brought to light the fact that we have a lot of people with food, transportation and other insecurities,” DeBoer said. “When you think about how it was exacerbated, it also motivated us even more.”

Looking at the vast expanse of Sanford’s rural footprint and care, DeBoer said not everyone has the same access to resources as those in big cities. The website then helps patients find more accessible resources where they are.

In the past, Sanford care teams have worked to find resources for patients, but DeBoer says this website allows them to better serve patients and gives them the ability to navigate those resources on their own. It also allows teams to provide resources to patients who may be outside of the communities in which they seek care.

As inflation continues to affect everything from gas prices to food and housing, DeBoer says the challenges some patients face may be exacerbated and affect how they maintain good health or take care of their family.

DeBoer added that the partnership between findhelp and Sanford also allows Sanford to address inequities in the community by partnering with existing services that meet patient needs.

“It’s really our opportunity to partner with our communities too.”

Through this partnership, findhelp manages the database for finding and verifying community resources across the Sanford footprint. This means anywhere there is a Sanford or Good Samaritan location.

Now, Daniels says that when a patient shows up for an appointment or contacts the healthcare system with a question, there will be plenty of resources available to help them find the answer that’s best for them.

“We provide world-class healthcare and we want to be able to connect our patients to the people and our partners in our community who also provide this type of care,” Daniels said.

Small businesses in St. Louis now have access to easier tools to help them establish an online presence.

Mastercard on Wednesday announced the rollout of a branded version of its Digital Doors website, alongside the Small Business Empowerment Center. Digital Doors helps businesses design and build websites and offers them discounts on platforms like Facebook Business and Shopify.

While all businesses can access digital doors online, engaging the Small Business Empowerment Center as a partner builds trust in the tools, said Ginger Siegel, head of Mastercard’s small business efforts in North America.

“We believe that St. Louis has a very unique relationship with its small businesses, and we believe that small businesses in St. Louis will be much more likely to want to go to organizations they know,” Siegel said.

The company estimates that at the start of the pandemic in 2020, as many as 10 million small businesses in America did not have an online presence. This number included Diversity Gallery, a clothing store and a natural hair salon in the Vandeventer district.

Gallery owner Leslie Christian used Digital Doors tools to update her website and set up online payments.

“These resources will allow me and my business to stabilize and be able to pivot from this pandemic,” Christian said. “I’ve endured a recession and now COVID, so I’m really optimistic about this opportunity to be able to take my business to the next level.”

A more dedicated bus service arrives at Pentagon City. Today, Arlington County officials and representatives from state and regional transit agencies broke ground on the site of the future bus terminal at 201 12th Street South to mark the first phase of the Transitway expansion project. .

The Transitway Extension will improve transportation needs in the Crystal City-Potomac Yard corridor in the National Landing area by adding seven new transit stations as well as additional dedicated bus lanes. These additions will improve connections to the Washington Metropolitan Area Transit Authority (WMATA) and Columbia Pike Metrorail stations and support planned residential and commercial developments at Pentagon City, Crystal City and Potomac Yard in the city of Alexandria.

The Transitway Extension is primarily funded through a partnership with the Northern Virginia Transportation Authority (NVTA) and the Virginia Department of Rail and Public Transportation (DRPT).

“This extension of the Transitway is critical as it will support an increase in demand and regional travel needs as Pentagon City and Crystal City continue to thrive with the arrival of new businesses such as Amazon Headquarters 2 and the Virginia Tech’s innovation campus in Alexandria, attracting commuters and visitors from across the region,” said Katie Cristol, Arlington County Board Chair and NVTA Board Member.

“Having multiple, reliable ways to get around the region is critical to realizing the county’s plans for an 18-hour city in Pentagon City and Crystal City, with a dynamic range of places to live, play and work, all easily accessible by public transport” Cristol added.

This extension adds 1.1 miles to the 4.5 mile corridor that currently connects Crystal City and Braddock Road Metrorail stations, which has been in service since 2016. WMATA will operate the bus service lines once the extension is complete.

“Once complete, this multimodal connection and Pentagon City’s transit expansion will serve as a connection and enhancement to Metrorail and economic activity throughout the region,” said Phyllis J. Randall, President of the NVTA and the Loudoun County Board of Supervisors.

“NVTA has contributed a total of $227 million in transportation projects to National Landing, including nearly $29 million in total investments in this Pentagon city alone and multimodal connections and the Transitway expansion, an investment for the region,” added Randall.

When complete, the Transitway Extension will directly benefit approximately 23,000 existing and planned residential units as well as 24.9 million square feet of existing and planned commercial space within one block of bus routes and dedicated stations. ‘by 2045. Building Updates

The letter Brown sent to the city council argued that the Langley extension would reduce green space in the city and hinder access to recreational areas. She also disputed that the Langley Parkway extension would improve ambulance response times and estimated the total cost to ratepayers at nearly $22 million.

Brown considers the wooded area the boardwalk would pass through as a valuable natural resource for Concord and something that can draw people to the city. “It’s an asset, and it’s something people in multiple neighborhoods use,” she said in an interview.

His goal, and the goal of other residents who oppose the project, is to remove Langley from the budget’s capital improvement program in the next budget.

The Langley Parkway project is listed as part of the current year’s capital improvement program for funding in fiscal years 2024 and 2026. Description of CIP #40 comes with a price tag of $12.5 million dollars, including a private donation of $2.8 million.

State law allows municipalities like Concord to create a capital improvement program for capital investments in facilities, infrastructure and equipment to be implemented in future years, allowing the council to plan budgets for the future and avoid sudden increases in tax rates.

Concord features 10 years of projects in the annual capital improvement program, a practice in place since fiscal year 2011. Over 100 projects are included in the current capital improvement program. Funding is only allocated to projects in the upcoming fiscal year.

Projects are added to the capital improvement program during the annual budget adoption process, which begins next week on May 12, when city council receives a copy of the proposed budget. A session on the capital improvement program is scheduled for June 2 and public hearings on the budget are scheduled for June 9.

Although the final section of Langley’s three-phase expansion returned to the public eye in April, the road has been part of the city’s long-term road planning for decades. Concord’s 2030 Master Plan refers to the construction of a northern section of Langley Parkway as “the largest and most needed upgrade to the city’s road network”.

A 2015 transportation feasibility study that estimated the cost at around $13 million to $15 million pointed out that the Langley expansion could create better emergency access to the hospital’s Level 2 trauma center and create a secondary access road in case Pleasant Street is blocked. He also said the new road could reduce congestion from cars crossing residential side streets to avoid busier routes.

Between 32 and 36 properties could be affected by the project, depending on the plan followed, according to the study. “The conceptual designs indicate that no full ownership acquisition will be required to accommodate the project, only small strip areas for (the right-of-way),” the Vanasse Hangen Brustlin, Inc. report said.

The city’s deputy director for development, Carlos Baía, said that although the project has been on the master plan for a long time, city staff have not been tasked by the council to review its implementation.

While some residents who signed the petition expressed concern that there had not been enough opportunities for public participation in the project, other councilors said they were frustrated with the “misconceptions” of opponents of Langley – including the idea that funding for the road would be approved without a transparent process.

“If, by any stretch of the imagination, this project were to go ahead, there would be a lot of public meetings and public hearings,” said At-Large Advisor Byron Champlin. Champlin called the money set aside for Langley under the capital improvement program “just a placeholder.”

“I don’t think we have the resources to move this forward right now. I think we have other pressing needs,” he said, including purchasing an additional ambulance and adding staff to the police department.

Another newcomer to the city council is Ward 4 Councilor Karen McNamara. Like Brown and Ward 6 Councilman Paula McLaughlin, this spring will be McNamara’s first budget cycle. She heard residents in Ward 4 worry that continuing construction of the Langley extension would mean cutting down trees or losing access to walking paths, but she stressed that council would have to approve all appropriations for the project.

“If it comes to a place where the city council is discussing it, I’m sure there would be more data that I would like to see as to why to consider this plan,” McNamara said. “It’s not even something the city council is working on right now.”

Arlington County’s park system is ranked third in the nation by the Trust for Public Land’s ParkScore® Index.

With 99% of Arlington residents living within a 10-minute walk of a park, the importance of county parks has been keenly felt during the COVID-19 pandemic. Arlington’s more than 150 parks served as places to connect and exercise, and they played a vital role in strengthening the mental and physical well-being of our community. In recent years, Arlington has ranked fourth in the nation.

“Our parks and recreation opportunities are a key contributor to the quality of life in Arlington County. From livability, bikeability, fitness, health and more, our community benefits from our parks,” said Arlington County Board Chair Katie Cristol. With fixed-route bus service, 135 miles of bike paths known as MCT Trails, and the RideFinders carpool and rideshare program, MCT offers a variety of greener transportation alternatives. MCT bus service in Madison County is just $1 each way and with service between communities and within communities, there are a number of great options for those who live and work locally.
